Evolution has made this process fairly efficient. Yet, sometimes, anomalous clotting occur in the bloodstream and these clots can block a blood vessel cause problems like stroke. To be more updated, I'd imagine that the clotting deluge would have to be more selective and own redundancy checks to eliminate the possiblity of clotting blood inside the body.
