Why would someone not know how to be aware of throbbing?

if they be unconscious, have anesthesia,and stuff like that.

there's a condition call CIPA -- congenital insensitivity to stomach-ache. It is one of the rarest conditions on the planet with smaller quantity than 100 cases documented. but when someone has this, they be aware of no pain, no sensitivity to fry, cold, etc.
